Register
Registration is simple: if you reside within our practice area, simply come into surgery and see a receptionist. She will give you a mauve form to complete for each family member, and give you an appointment to see the nurse for a New Patient check. Please remember that it takes a few weeks for your old notes to arrive from your previous GP so if you have details with you, such as repeat prescription lits etc. this will ease the transition period.
Make a complaint
We try to do our best at all times, but we will never be perfect and want to learn from our mistakes. If you have a complaint or just a helpful comment, please address it to the Practice Manager, Stella Thake, either verbally or in writing - we would prefer it to be signed so that we can make a response personally. If you feel that we have not addressed your concerns adequately, you should address them to the South Wiltshire Primary Care Trust.
